Season 2 didn't have many markerbob episodes, but in season 3, just about every other episode was markerbob. Although markerbob was the most common style used in season three, there were still more episodes that had normal animation.
the perfume department scene in shanghaied was full on markerbob

Patrick stayed the same from start to the beginning (not including cel prototypes)
 
I think they color the outlines every opportunity they get
MovieKrabs and i think Movieward (the real ones not the season 4 knock offs) uses colored outlines alot, however markerbob uses the colored outlines sparingly

No, not really, the shape of Spongebob's head still gave it away. His head is bigger at the top than it is on the bottom in the early and middle markerbob episodes, in full on Markerbob his head is perfectly square.

No, you'd be surprised, markerbob has more colored lines than any other style used in the series. Rewtach some markerbob episodes and you'll see what I mean. As for the least amount of colored lines, I'd say season 1, but that's because it's cel animated.

I've noticed that normal episodes have animation smears, but markerbob doesn't. How does rough draft studio know whether or not to make an episode normal or markerbob?
